Exploration des représentations, opinions et attitudes des médecins généralistes face aux spécificités de santé des patients issus de la communauté lesbienne, gay et bisexuelle : étude qualitative sur le territoire de la Gironde

Abstract : Introduction: LGB patients are displaying specific features regarding their health, which implies that their care must be adapted. These last years, several studies have demonstrated that general practitioners do not bring an appropriate answer to the specific needs of sexual minorities. There is a lack of understanding of their biomedical, and also psychosocial, characteristics. The main objective of this study is to investigate the different approaches of the general practitioners from Gironde to manage the health of sexual minorities. The secondary objectives are to evaluate the stakes of addressing sexual orientation in the general practice and to investigate general practitioners’ perceptions on the tools regarding LGB health (LGB friendly lists, community health centers, and LGB health training). Materials and methods: Qualitative study by semi-directive individual interviews. Collection made from general practitioners from Gironde. Maximum variation sampling. Results:eEight general practitioners were interviewed. Most of them are using a universal approach, sometimes making some adjustments in their care to LGB patients. The community-based approach is seldom used by fear of a separatist drift. The existing tools for LGB health are seen by some as a necessary response to sexual minorities’ health disparities, and by others as the origin of a social division. The approach of sexual orientation in general practice is one of the first impediments to comprehend the specific needs of LGB patients. The various factors affecting its approach depend mainly upon personal and professional representations from the part of primary care physicians. Discussion: to better understand the health specifics of sexual minorities, physicians can decide to adopt a community-based approach. It is important to take into account the psychosocial context LGB patients are living in, including the minority stress, in order to give them a comprehensive care.
