Limites à la prise en charge des adultes obèses en médecine générale : enquête auprès de médecins généralistes des Pyrénées-Atlantiques et pistes d’amélioration

Abstract : Introduction: obesity is a public health problem concerning 17% of French population, its management by the care system is essential. Aim: the aim of this study was to describe the obstacles of general practitioners in the Pyrénées-Atlantiques to the care of obese patients, and to provide avenues for improvement. Method: we carried out a qualitative study with semi-structured interviews among fourteen general practitioners in the department. These interviews were recorded and then transcribed in the form of verbatims. A thematic analysis with double coding was carried out. Results: for doctors, obesity is a multifactor pathology which requires multidisciplinary management. Despite a fairly systematic approach in consultation, the general practitioner lacks time and training for efficient care. The development of the care network should be encouraged, as physicians find it a real help for patients. Patient motivation is essential, and must be reinforced by strong societal measures so as not to stigmatize the person suffering from obesity: prevention of childhood obesity, reduction of advertising and screen time, promotion of physical activity, measures with food companies (e.g. Nutriscore label). Conclusion: this study clarified the limits of general practitioners in the Pyrénées-Atlantiques on the management of obesity. This could be improved by strengthening a multidisciplinary approach as well as by more incisive societal measures.
