Evaluation of clinical practices for early HER2-positive breast cancer in Côte-d’Or : a retrospective study based on the breast and gynecologic cancer register of Côte-d’Or
Évaluation des pratiques de prise en charge des cancers du sein localisés HER2 positif en Côte-d’Or : étude rétrospective à partir du registre des cancers du sein et gynécologiques de Côte-d’Or

Background : Standard care for early HER2-positive breast cancer is based on surgery associated with chemotherapy and HER2-targeted therapy (Trastruzumab). Before 2019, the chemotherapy was administered before surgery only to allowed a breast-conserving surgery by downsizing the tumor. Since, neoadjuvant therapy has become the standard of care because it provides information about tumor response in vivo and patient who did not achieve a pathological complete response (pCR) will benefit of TDM-1 after surgery. The aim of these study was to describe the clinical practices for those patients and to identify a change since 2019.
Method : We performed a retrospective study based on the breast and gynecologic cancer register of Côte-d’Or involving the patients with an early HER2-positive breast cancer between 2005-2015 and 2019-2020.
Results : From 2005 to 2015 we included 480 patients. The neoadjuvant chemotherapy rate was 14% and the pCR rate was 54%. Association of taxane and Trastuzumab was the main chemotherapy protocol for neoadjuvant therapy (65%). The overall survival was 83% with a statically significant higher rate for patients who achieved pCR (HR= 0,2 IC95% (1.06-23.7), p= 0,02).
From 2019 to 2020 we included 63 patients. The neoadjuvant chemotherapy rate was 73% and the pCR rate was 37%. Association of anthracycline, taxane and Trastuzumab was the main chemotherapy protocol for neoadjuvant therapy (91%); after surgery the majority of patients received taxane and Trastuzumab (75%).
Conclusion : We observed a real change in clinical practice for early HER2-positive breast cancer both in therapeutic sequence and chemotherapy protocol since 2019.
Introduction : La prise en charge standard des cancers du sein localisés HER2-positifs repose sur une association de chirurgie, chimiothérapie et thérapie anti-HER2 (Trastuzumab). Cette chimiothérapie était réalisée en néoadjuvant (CNA) jusqu’en 2019 dans le seul but de permettre un geste conservateur du sein. Depuis, le test de chimio-sensibilité tumorale qu’elle procure permet une amélioration du pronostic des patientes non en réponse histologique complète (pCR) par une intensification thérapeutique (TDM-1 adjuvant). L’objectif de cette étude était de réaliser un bilan des pratiques de prise en charge pour ces patientes et d’identifier une modification depuis le changement de paradigme de 2019.
Méthode : Il s’agit d’une étude rétrospective observationnelle réalisée à partir des données du registre des cancers du sein et gynécologiques de Côte-d’Or incluant les patientes prises en charge pour un tumeur du sein localisée HER2-positive entre 2005-2015 et 2019-2020.
Résultats : Sur la période 2005-2015, 480 patientes ont été incluses avec un taux de CNA de 14% et de pCR de 54%. Un protocole taxane, Trastuzumab était utilisé pour la majorité des CNA (65%). La SG était de 83% et significativement meilleure chez les patientes en pCR (HR= 0,2 IC95% (1.06-23.7), p= 0,02).
Sur la période 2019-2020, 63 patientes ont été incluses avec un taux de CNA de 73% et de pCR de 37%. Le protocole anthracycline, taxane, Trastuzumab était utilisé en majorité pour la CNA (91%) et le protocole taxane, Trastuzumab pour la chimiothérapie adjuvante (75%).
Conclusion : Un réel changement de pratique s’est effectué depuis 2019 tant sur la séquence thérapeutique que sur les protocoles de chimiothérapie pour les patientes atteintes d’un cancer du sein localisé HER2-positif.
