Développer l’éducation thérapeutique en soins primaires : à propos de TyMobilETP, organisation d’appui des structures de santé en zones prioritaires ; rôle auprès des professionnels de santé

Abstract : Context: Therapeutic Patient Education (ETP) has been met with reservations regarding its implementation in primary care. Its development has encountered many obstacles. In order to counteract these obstacles, it seems important to devise a better adapted format that could bring General Practice professionals to adopt a pedagogical point of view. TyMobilETP is a local experimental project led by a mobile resource team aimed at training professional health care workers on the ground to help them adopt a educative attitude. Goal: Investigate how practices and representations of health care practitioners have changed as far as ETP is concerned after the implementation of TyMobilETP at the health care center in Mauron. Methodology: Qualitative study with semi-guided interviews of professional health workers at the health care center in Mauron. Results: After the intervention of the TyMobilETP team, professionals became more familiar with the concept of ETP which they did not see as being as encompassing before. For some, this concept comforted them in their vision of healthcare, whereas for others, it seemed a disturbing concept. In their daily practice, they emphasize an attempt at reexamining their interaction with the patient and a tendency to listen more. All of them described ETP as teamwork and all of them expressed a desire for interprofessional collaboration. They all insist that obstacles still exist for the collaborative implementation of ETP on the ground: lack of time for training in the case of independent health care professionals, lack of involvement in the case of doctors, the rural environment and a search for leadership and collective intelligence. Discussion: TyMobilETP has initiated an identity transformation with health care professionals, which is worth supporting. It is also a catalyst for the development of interprofessional collaboration. There are some lingering obstacles which seem possible to overcome. What is needed is shared local leadership, collaborative work between health care professionals in order to emphasize each individual’s willingness to act, creating collective intelligence and building a project with all professionals and all patients together.
