Intérêt de la mise en place d'une base de données pour la gestion personnalisée du capital sanguin en périopératoire de chirurgie orthopédique au CHU de Caen : étude rétrospective, observationnelle

Abstract : Introduction: The objective of our work was to estimate the long-term benefit in the use of a large database for the implementation of a PBM protocol on the evolution of blood transfusion in primary total knee arthroplasty (TKA) in women. Type of study: Retrospective, observational, single-center study. Patients and methods: 1364 patients operated on for TKA were included between January 3, 2011 and December 14, 2020. During this period, the evolution of the management of perioperative anaemia, blood loss on the first day (D+1), transfusion rate, and average length of stay were analysed. Results: The prevalence of D-1 anaemia decreased from 24.5 to 19.2% between 2011 and 2020, the rate of EPO and IV iron prescription after 2017 was 79.3 and 62.3% respectively. The prescription rate for acid tranexamic increased from 41.9% to 88.1% between 2011 and 2020. Median blood loss decreased from 326 [236 - 417] to 230 [149 - 315] from 2011 to 2020. The median length of stay decreased by 4 days between 2014 and 2020. Conclusion: The PBM protocol established in our department as well as the daily follow-up of patients through a database allowed a rigorous management of patients undergoing TKA with the benefit of a perioperative reduction in blood transfusion. and the length of stays.
