Comparaison des infections à SARS-CoV-2 : formes sévères versus non sévères chez le sujet âgé lors de la seconde vague de l’épidémie

Abstract : Introduction:Corona Virus Disease 2019 (COVID-19) is responsible for a pandemic since 2019. During its second wave, people over 75 years of age accounted for 47 % of hospital admissions and 75 % of deaths. We looked for predictive factors of severe illness in this population. Methods: it is a case-control, retrospective, observational, monocentric study performed in the COVID ward of Cimiez hospital, in Nice. We collected clinical, biological, and imaging data from patients on the 3rd, 7th, and 14th days following infection. Results: we included 215 COVID-19 positive patients, 41 % of which suffering from a severe form. Subjects were mainly men (54 %), and the mean age was 87 ± 6 years. Predictive factors for severe form were : obesity (19 %, p = 0,002), dyspnea (92 %, p < 0,001), oxygen saturation < 90 % (90 %, p < 0,001), impaired consciousness (51 %, p < 0,001), C-reactive protein (CRP) > 80mg/L (p < 0,001) and lymphopenia (p < 0,001). so-resources group ≤ 3 (31 %, p = 0,002) and neurocognitive disorders (39 %, p = 0,001) are less frequent in severe forms. Conclusion: we highlighted predictive factors for severe forms of COVID-19 in the elderly population. It would be interesting to create a prognostic score combining clinical and biological data.
