Parcours de soins des personnes âgées consultant aux urgences de Saint-Benoît : étude d’une population évaluée par l’équipe mobile de gériatrie

Abstract : Background : Younger but ageing more rapidly than in mainland France, the population of Reunion Island is also in poorer health as it is more likely to remain at home. The current trend is to limit the number of emergency room visits by the elderly. Communication between hospital and primary care is fundamental and the mobile geriatric teams are one of the main elements. The aim of this study is to describe the dependent elderly population that visits the emergency department and their care pathway. Méthod : This is a descriptive observational study. The retrospective data collection is carried out on elderly patients in the Saint-Benoît emergency department in 2019 for whom the mobile geriatric team intervened. Résults : This dependent population (GIR 2), 82.5 years old on average, predominantly female (56%), living with others (66%) at home (95%), often consulted for an altered general condition in a stable state (87.5%). Admission was often considered irrelevant (52%). Loss of autonomy is the most frequent geriatric syndrome. Discussion : The main problems raised are the growing isolation associated with loss of autonomy, the bypassing of the general physician, the lack of a communication between hospital and primary care symbolised by the mobile geriatric team, and the too late management of dependency. Future challenges will concern the coordination of the ambulatory care within a pathway logic, the optimisation of the role of the mobile geriatric team and the reinforcement of prevention.
