Évaluation des temps de prise en charge des patients consultant aux urgences du CHU de Caen pour douleur thoracique et ayant bénéficié d'au moins un dosage de troponine : troponine délocalisée et troponine hypersensible : une étude rétrospective monocentrique

Abstract : Objective: Determine if the implementation of delocalized troponin in the emergency department of Caen University Hospital modifies the medical management times of patients consulting for chest pain. Material and method: A retrospective single-center study including 708 patients consulting at the emergency department of Caen University Hospital for chest pain and having received at least one troponin assay, was performed on the periods of May and June 2019 and May and June 2020. The primary endpoint was the comparison of overall medical management times of patients. Results: 351 patients were included in the “high-sensitivity troponin” group and 357 patients in the “delocalized troponin” group. 143 patients in each group received a single troponin assay. There was no significant difference in overall medical management times. 208 patients received a high-sensitivity troponins cycle and 214 patients received a delocalized troponins cycle. We demonstrated a significant difference in overall medical management times, with a decrease of 99 minutes of the transit time in the “delocalized troponin” group. Conclusion: The delocalized troponin implementation in the emergency department of Caen University Hospital decreased the overall medical management times of patients receiving a troponin cycle. There was no significant difference in the management times for patients receiving a single troponin test. However, on a day-to-day basis, the delocalized troponin contribution seems to be significant, with a reduction of "overcrowding", particularly in the vital emergency sector.
