Comment organiser son officine pour mieux accompagner les patients vivants avec une maladie chronique : retour d'expériences de quelques pharmacies

Abstract : In France, patients with chronic diseases occupy an increasingly important place in society. Pharmacists are therefore called upon to become more involved in the care of these patients. In recent years, they have been able to do pharmaceutical interviews, shared medication reviews or participate in a therapeutic education program to better support these patients living with a chronic disease. However, many obstacles to the development of these missions have been highlighted. First of all, pharmacists lack time, do not have enough staff and do not have suitable premises. Then, the remuneration of these missions does not seem to match the amount of work required. In addition, some of them were confronted with difficulties to set up an interdisciplinary cooperation with doctors or a difficulty to recruit non-requesting patients. Finally, some of them just don't see the point. Through the testimony of various pharmacists who have succeeded in setting them up and making them last, it has been possible to highlight the various strategies adopted by them in order to overcome the obstacles encountered during the implementation of these missions. The aim of this work is therefore to provide concrete solutions to pharmacists wishing to invest in these new missions. Thus, we mainly find changes at the level of the premises, the realization of training, the proposal of a more global approach to the patient, of teamwork, the establishment of a quality system, etc. This work also responds to how to introduce them to patients, how to better manage time's work, what about remuneration and how to collaborate with doctors on these missions
