Évaluation de la réponse au remplissage par le doppler carotidien au bloc opératoire du CHU de la Réunion

Abstract : Background : Predicting fluid responsiveness is a major issue in anesthesia. Many static and dynamic indices have been used, but some are invasive. The aim of this study is to determine if carotid blood flow doppler can correctly assess cardiac response to a mini fluid challenge. Method : We prospectively recorded 58 measures in ventilated patients who received general anesthesia. Velocity time integral variation (%VTI250 and %VTI100) and systolic velocity variation (%Vc250 and %Vc100) derived from carotid Doppler analysis and stroke volume registered in esophageal doppler before and after a mini fluid challenge of 100- and 250-ml saline. Results : Increase of %VTI250 12,2% or higher and %Vc250 8,5% or higher after an mini fluid challenge showed a sensitivity of 76% and 63%, a specificity of 75% and 70%, and an area under the curve of 0.79 and 0.68, respectively, for discriminating responder and nonresponder patients. According to Pearson coefficient, %VTI250 and %Vc250 were found to be positively correlated with esophageal doppler stroke volume variation (r² = 0.42 and 0,094, respectively; P <0 .001 and 0,48) and were significantly different between responder patients and nonresponders. Conclusion : Variation of carotid Doppler parameters before and after mini fluid challenge mirrors cardiac response to fluid loading. This tool could be considered as an alternative to esophageal doppler in a general population of anesthesia.
