Étude SENITOX : effets indésirables graves des chimiothérapies adjuvantes et néo-adjuvantes du sujet âgé pris en charge pour une néoplasie mammaire

Abstract : Introduction: It is noted that 10% of cancers occur in the population greater than 85 years of age. Breast cancer, the most common type of female cancer, with the majority occurring in the over 70 age group. With the ageing population, it has become necessary to weigh the benefits against risks of chemotherapy. Methods: This monocentric retrospective study included all patients of at least 70 years old with localized breast cancer who received adjuvant or neoadjuvant chemotherapy. The primary endpoint was to describe serious adverse effects (SAE). This was defined by at least grade 3 toxicity as stated in the Common Terminology Criteria for Adverse Event. The second endpoints were to study the consequence of a SAE on treatment and to identify the risk factors of SAE. Results: From January 2015 to July 2020, 216 patients were included with a median age of 73. In 38% of our patients, a SAE was reported, 89% of them of grade 3, leading to a hospitalization in 40% cases. Main SAEs included infections (19,5%) and blood disorders (16%). If a SAE occurred, this led to chemotherapy being stopped in 41,5% and to modification of treatment plans in 40% of patients. Two patients had a lethal SAE. In multivariate analysis, increased creatinemia was associated with higher risk of SAE (OR=1,25, 95%IC [1.04-1.51], p=0,021). More than 80% of patients were in complete response. Conclusion: Even with an undeniable benefit of chemotherapy in localized breast cancer treatment, our study highlights a poor tolerance with increased risk of severe toxicities. Treatment decision in elderly patients should be included in global evaluations.
