Impact du remplissage initial sur le pronostic de la pancréatite aiguë

Abstract : Introduction: The treatment of acute pancreatitis (AP), which is one of the main causes for hospitalization in Gastroenterology, is based on fluid rescuscitation, nutrition and organ failure support. The modalities of fluid rescuscitation are poorly codified and studies on its impact are contradictory. Few studies have looked at the impact of the delay of perfusion start. Objective: The aim of this work is to evaluate the impact of the delay of initiation of infusions and the initial fluid volume on the course and the prognosis of AP. Material and method: We conducted a single-center retrospective study including patients with AP at Caen University Hospital between January 2018 and May 2020. We recorded the rate of severe forms and complications (death, transfer to intensive care, infection) according to the delay of infusions (before/after 3 hours) and to the volume of fluid resuscitation in the first 24 hours. Results: A total of 293 patients were included: 5.8% were severe. The mean delay of infusions was 3.2 hours and the mean volume at 24 hours was 0.75 mL/kg/h. The rates of severe forms or complications were not different according to the delay of initial infusion (6.1% of severe forms in the "infusion before 3 hours" group versus 5.3% (p=0.78) and 9.4% of complications versus 8% (p=0.66)) or according to the initial fluid resuscitation volume (6.9% of severe forms for a volume >1.5 mL/kg/h versus 5.3% (p=0.67)). Conclusion: In our study, the delay and the initial volume of the infusions did not seem to impact the occurrence of a severe form of AP.
