Imagerie des manifestations des hémopathies malignes au niveau du système nerveux central chez les enfants : formes révélatrices, récurrentes et complications

Abstract : Purpose: to document the radiologic abnormalities seen in the central nervous system (CNS) in childhood malignant hemopathy (leukemias and lymphomas) or as complications of its treatment. Materials and methods: Magnetic Resonance images and Computed Tomographic (CT) scans were reviewed retrospectively in 73 children and adolescents with neurologic manifestations/complications of malignant hemopathy or its treatment. Patients were divided in two groups. The first group included those with disease-related complication of malignant hemopathy, such as parenchymal, meningeal and bone infiltration. The second group included patients with treatment-related neurotoxicity and infection caused by immunocompromised states. Factors that predisposed to the development of tumor-related or treatment-related complications were determined by reviewing the medical records.Results: the first group included 30 patients, 19 had direct infiltrations revealing hemopathy, 9 had direct infiltrations revealing a disease relapse and 2 cerebral haemorrhages due to leukostasis. The second group included 43 patients. The imaging abnormalities seen are sinus thrombosis (n = 17), toxic leuco-encephalopathy (n = 12), PRES (n = 5), cerebral infarct (n = 2), infections (n = 4), lumbar adhesive arachnoiditis (n = 1), smaller white matter volume (n = 1) and Ophelia syndrome (n = 1). Conclusion: the wide spectrum of CNS abnormalities that occur during and after treatment for malignant hemopathy is related to the inherent risk of the hemopathy itself, to the treatment method, and to the duration of survival. Because many neurologic complications of hemopathy are treatable, early diagnosis is essential.
