Association du vitiligo et psoriasis : cohorte descriptive rétrospective de 74 patients

Abstract : Background: vitiligo and psoriasis are common chronic inflammatory skin diseases that could be associated in the same patient. However, this association remains mainly descriptive in the literature. Objective: to examine the demographic and clinical characteristics of patients with the occurrence of psoriasis and vitiligo Methods: single center cross-sectional study including consecutive patients consulting for vitiligo between January 2015 and January 2020. Demographic and clinical features of patients with vitiligo or the co-occurrence of psoriasis and vitiligo were reported Results: four hundred and thirty-six patients with vitiligo patients were included in this study, 74 vitiligo patients (45 women) had a past and/or current personal history of psoriasis. Thirty-six patients (48.7%) had a generalized vitiligo pattern with a mean VES score of 3.9 (σ = 6.8) The plaque-type was the most common (n = 48–64.8%) with a mean PASI score of 1.875 (σ = 1.5). Psoriasis lesions were located at the same site as vitiligo in 12 patients (16.2 %). The mean age at psoriasis onset was 28 years (σ = 14.6), and the mean age at vitiligo onset 34.6 years (σ = 17.3). The occurrence of psoriasis before the development of vitiligo was reported by 46 patients (62.2%). In the multivariate logistic regression analyses, patients with both psoriasis and vitiligo had an independently higher risk of autoimmunity (OR: 11.8; 95%CI: 3.4–41; p < 0.001) and of a family history of psoriasis (OR: 2.4; 95%CI: 1.02–5.77; p = 0.046). However, in patients with a history of psoriasis, there was no independent association with a personal history of atopy (OR 0.3; IC 95% [0.12–0.82]; p = 0.018). Conclusion: this study adds better insights of demographic and clinical characteristics of patients with the co-occurrence of psoriasis and vitiligo.
