Influence de la patientèle médecin traitant sur la gestion des soins non programmés

Abstract : Context: Unscheduled care (UC) corresponds to a request for consultation, whatever the reason, for the day or the following day, addressed to a general practitioner during opening hours. The management of UCs is a current issue that might be partly responsible for the saturation of emergency departments in France. Objective: To determine the influence of the size of the attending general practitioner's patient base on the management of unscheduled care in Auvergne Rhône-Alpes. Methods: A self-questionnaire was sent by the URPS-AuRA to all general practitioners (GPs) in Auvergne Rhône-Alpes who had an email address in February 2021. The questionnaire recorded the sociodemographic characteristics of the GPs, the organization of their practice and finally the quantity of unscheduled care carried out during the week of January 11th 2021. Results: 220 questionnaires were analyzed. The average patient base of the general practitioners was 1086 and they performed an average of almost 4 unscheduled care consultations per half day. The larger the GPs patient base was, the greater the number of unscheduled care consultations were performed. The absence of a specific practice pattern and an insufficient supply of general practice in the practice area were also associated with more UCs visits performed. Conclusion: The size of a GP's patient base has a positive influence on the number of unscheduled care visits performed. However, there are still many criteria (known or unknown) that also influence the number of UCs performed.
