Place de l’EEG d’amplitude dans la prise en charge de l’accident vasculaire cérébral ischémique du nouveau-né : étude rétrospective entre 2010 et 2020 en Normandie Occidentale

Abstract : Neonatal arterial ischemic stroke is rare but potentially provider of handicap with cerebral palsy in 30% of patients, sometimes associated with epilepsy. Neonatal arterial ischemic stroke reveals by hemicorporal seizures in the first week of lite and is confirmed by an RMI. The actual medical care is to treat seizures. An amplitude integrated EEG (aEEG) seems to be interesting to manage and treat better those seizures and to allow a reading of the EEG trace 24h/24 in bed. This study’s goal was to describe the population of newborn treated in Occidental Normandy for an arterial ischemic stroke in 10 years (2010-2020) and to analyze aEEG ‘s monitoring effect. Thirty-eight newborns were treated for an arterial ischemic stroke during this period. We have had significal results: newborns monitored by aEEG are less treated before EEG trace than the others (3 newborns vs 16, p<0,01), the inborns are less treated before EEG trace as well (6 newborns vs 13, p<0,01). We noticed a change in practices in years with less newborn treated before EEG trace, less newborn treated as discharge and the emergence of a new antiepileptic drug: levetiracetam. We noticed a tendency in the speed of recording the first EEG trace in newborn monitored by aEEG than the others. Results in this retrospective study on newborns treated for arterial ischemic stroke are getting closer to those find in literature. We have to insist on importance of a long term neurodeveloppemental follow up of these children, for the screeing of neurodevelopemental troubles wich requires forward interventions, as weel as the long-term resounding of new antiepileptiq medications used.
