Prise en charge de la thrombose veineuse profonde en soins primaires : une approche qualitative

Abstract : Introduction: Deep vein thrombosis (DVT) is a pathology that is managed on an outpatient basis. Studies show that the recommendations are taken up differently. The aim of this work was to identify the determinants of DVT treatment in primary care and to characterize the care channels mobilized. Materials and methods: A reasoned sampling witch maximum variation allowed the recruitment of twelve Norman general practitioners, who were interviewed by semistructured interviews during the month of April 2021. The verbatims followed by a general inductive analysis, according to thematic and experiential axes. Results: The main determinants of the management of DVT in primary care are clinical intuition, the implementation of alternatives means, the safety of the doctor and the patient within the framework of an optimal doctor-patient relationship and the local medical environment allowing optimal care coordination. The care channels mobilized are said to be proximity, and dependent on the work habits and interactions of participants with organ specialists. Conclusion: In primary care, general practitioners report difficulties in evaluating the duration of treatment. The popularization of the specialty of vascular medicine and the place of the vascular doctor could allow the creation of local care channels and the optimization of care.
