Grippes graves prises en charge en réanimation à la Réunion de 2010 à 2019

Abstract : Background : The severe forms of influenza infection requiring ICU admission remains a medical challenge due to its high mortality. New H1N1 strains were hypothesized to increase mortality. We present a large series focusing on ICU-admitted influenza patients over the last decade with an emphasis on factors related to death. Methods : We conducted a retrospective study of patients admitted in ICU for influenza infection over the 2010-2019 period in Reunion Island (a French overseas territory). Demographics data, underlying conditions and therapeutic management were recorded. Univariate and multivariate analyses were performed to assess factors related to ICU-mortality. Results : 350 adult patients were analyzed. Overall mortality was 25.1%. We found age over 65 and cancer history as independent factors of death. Need for intubation, early intubation within 48h after admission, invasive MV, ARDS, vaso-drug support, ECMO, dialysis, bacterial coinfection, leucopenia, anemia and thrombopenia were related with a higher mortality in the univariate analysis. History of asthma and oseltamivir therapy were correlated with a lower mortality. H1N1 did not impact mortality. Age over 65 years was the unique factor independently increasing mortality in the multivariate analysis. Conclusions : Patient’s underlying conditions influence hospital admission and secondary ICU admission but were not found to independently impact ICU mortality except age over 65. Pulmonary involvement was often present, required MV and often evolved towards ARDS. We recommend rapid ICU admission of patients with influenza-related pneumonia, management of bacterial coinfection and early administration of oseltamivir.
