Le dépistage multiparamétrique de la maladie vasculaire placentaire au premier trimestre de la grossesse au Centre Hospitalier Ouest Réunion

Abstract : Objective of the study: Placental vascular diseases include pathologies obstetrical problems thought to be related to placental ischemic origin, such as preeclampsia, eclampsia, retro-placental hematoma, growth retardation in utero and fetal death in utero. They are a major cause of morbidity and mortality maternal and neonatal. For twenty years, early detection has been proposed by the Fetal Medicine Foundation according to an algorithm taking into account maternal, biophysical and biological characteristics. We describe the population benefiting from this multiparametric screening at the Hospital Center Ouest Réunion, named PRIMA FACIE, and we are studying the performance of this screening in the early prevention of placental vascular disease. Material and method: The PRIMA FACIE is a screening and prevention of placental vascular disease set up at the Hospital Center Ouest Réunion, accessible to all pregnant women consulting between 11 SA and 13+6SA. We compare the performance of this screening to a population with gave birth at the Center Hospitalier Ouest Réunion but did not benefit from PRIMA FACIE screening in the first trimester of pregnancy. Result: 794 pregnant women benefit from PRIMA FACIE between April 1, 2017 and November 30, 2019. We observe that patients consulting at the Center Hospitalier Ouest Réunion from the first trimester have more risk factors for develop placental vascular disease. This study shows the performance screening by PRIMA FACIE, reducing the risk of MVP of a population initially more at risk, at a risk equivalent to that of the general population (p=0.01). Conclusion: The results of the PRIMA FACIE protocol show the feasibility of this care approach in everyday practice, and its performance in terms of detection and prevention of placental vascular disease.
