Place du médecin généraliste dans la pré-exposition prophylaxie au VIH : une étude descriptive transversale à Paris

Abstract : Pre-exposure prophylaxis "PrEP" is a new prevention tool in the fight against HIV which has been used for high-risk populations since 2016 in France. Monitoring patients with PrEP is a key issue in general practice. The objective of the study is to assess the role of the general practitioner (GP) in the monitoring of PrEP with patients consulting in CeGIDD centers based in Paris and to address subsequent hindrance to this new process. Our study was descriptive and multicentric. The study was conducted in four Parisian sexual health centers from May to July 2019. Patients consulting within the framework of PrEP were presented with a paper self-report questionnaire. The primary endpoint was the mention of this treatment to his MG. A total of 201 subjects were included, out of which 165 patients consulted to monitor their treatment. 48% (n= 77) filled their MG in on taking PrEP 7% (n = 12) had already consulted their GP for the renewal of their treatment. 98% of the patients were MSM. In the group that "reported to their GP", the number of GP consultations during the year was significantly higher, and questions about sexuality were more frequent as well as discussions relating to HIV and its screening (p<0.0001). The refusal to discuss sexual topic is significantly higher in the group that "did not report to their GP". The GP role with PrEP patients remains limited. GP training and their collaboration with CeGIDD needs to be strengthened. Addressing sexuality in consultation is essential in building a relationship with patients that is based on trust.
