Pertinence du dosage des interleukines 6 et 10 dans les fluides intraoculaires pour le diagnostic de lymphome vitréo-rétinien : retour sur 8 ans d'expérience

Abstract : Purpose: To determine the diagnostic relevance of interleukin 10 (IL-10), IL-10/IL-6 ratio and previously published Interleukin Score for Intraocular Lymphoma Diagnosis (ISOLD) in aqueous humour (AH) and vitreous in classifying vitreo-retinal lymphoma (VRL) versus uveitis. Design: Single-center retrospective cohort study. Methods: Patients with a suspicious of VRL for whom IL-10 and interleukin 6 (IL-6) in the AH and/or in the vitreous had been measured were included. Patients were distributed in two groups: 13 ocular samples of 10 patients with VRL and 33 ocular samples of 28 patients with uveitis. Results: In patients with VRL, IL-10 median value was 118,0 [28,0; 217,0] pg/mL in AH. In patients with uveitis, IL-10 median value was 2,5 [2,5; 2,9] pg/mL in AH. A cut-off of 30 pg/mL in AH was associated with a sensitivity of 73% and a specificity of 96%. In AH, IL-10/IL-6 ratio higher than 1 and ISOLD were associated with the same sensitivity and specificity, respectively of 73% and 96%. An optimal cut-off of 0,67 was deduced from the receiver operating characteristic (ROC) curve of IL-10/IL-6 ratio in AH and/or vitreous. IL10/IL-6 ratio higher than the optimal cut-off of 0,67 was associated with a sensitivity of 92% and a specificity of 97%. Conclusion: IL-10 measurement, IL-10/IL6 ratio and ISOLD in ocular samples are useful to classify VRL versus uveitis. Cytology remains the gold standard for diagnosis, but intraocular cytokines analysis is a relevant additional tool for the diagnosis of VRL.
