Facteurs limitant et renforcant le recours à la PrEP des femmes en situation de prostitution de l’arc alpin

Abstract : The Pre-exposure prophylaxis (PrEP) it's recent tool to prevent the HIV. This prevention method carried by the MSM community remains very anecdotal among sex workers in France, of the 24,000 people taking prep regularly, less than 300 are women. Based on this observation, this study aims to understand facilitators and barriers to HIV pre-exposure prophylaxis (PrEP) for sex workers in the Alps This is a qualitative exploratory study based on conducting semi-structured interviews with sixteen women in prostitution between September 2020 and October 2021. The quantitative data was analyzed with Excel, the qualitative analysis was carried out by a single person who is also the principal investigator of the study. The epidemiological data are comparable to those in the literature. They shed light on the relationship between administrative and financial weaknesses on risk taking during the practice of prostitution.The qualitative analysis finds as the main barrier to PrEP the fear of judgment by the actors of the medical world as well as the mode of administration of PrEP. Among the main facilitators we find a strong feeling of protection of one's body and loved ones as well as a reduction in the fear of HIV infection. Finally, it provides some answers to improve access to PrEP, by rethinking more targeted modes of communication to the different populations practicing prostitution, and by implementing community health actions. To conclude, improving access to PrEP among sex workers requires a new way of thinking about communication towards the different target populations, but also a change in the mode of administration of PrEP. The long-acting injectable form would make it easier to start a PrEP treatment process. As such, a study evaluating the feasibility and improvement of access to PrEP by cabotegravir is justified.
