Évaluation des pratiques de l'enseignement de l'autopalpation mammaire par les médecins généralistes bas-normands

Abstract : Breast self-examination (BSE) is not recommended as its clear benefit has not been shown; however, according to the French Health Authority, about 50% of breast cancers in France are diagnosed in presence of clinical symptoms. The aim of our study was to investigate opinions about BSE among general practitioners (GPs) and describe the teaching methods they use to educate their patients. We carried out a quantitative study in the form of questionnaires sent to GPs in ex-Lower Normandy between 5th April and 11th June 2021. 70% of the doctors teach BSE to all their female patients regardless of age or medical history. Most of the GPs (54%) recommend an annual or multi-annual surveillance instead of a periodic exam as recommended previously. The main impeding factors described by the doctors are forgetfulness (49%), lack of time (43%), absence of evidence regarding the BSE's effectiveness or its anxiogenic character (34%) but also a feeling of unease or insufficient skills of the doctor (25%). Despite the absence of a clear benefit of BSE, the technique is taught by most GPs in ex-Lower Normandy in the form of a loose surveillance proposed to all female patients regardless of age or medical history. Potential impeding factors could be mitigated by additional medical training. Another solution would be the establishment of a breast cancer prevention consultation, which has been suggested by the French National Cancer Institute and is viewed positively by 90% of the doctors in our study.
