Attentes et représentations des médecins généralistes sur la prise en charge de l’adolescent en surpoids en ambulatoire : réflexion sur l’élaboration d’un outil

Abstract : Objective: bring out general practitioners expectations and representations related to the management of teenager overweight in order to discuss the interest of a dedicated tool. Method: this was a qualitative study, by semi-directed individual interviews, with thirteen doctors from the Pyrénées-Atlantiques enrolled by the “snowball” method. An interview collection was created based on scientific littérature. The experience and practice of physicians were collected and the development of a work instrument and its acceptability discussed. Their transcriptions until saturation were carried out on the N'Vivo software. Results: 600 minutes of interviews were transcribed into 1645 verbatims during thirteen interviews over December 4, 2020 to April 15, 2021. Doctors equated overweight to an underdiagnosed symptom treated too late. They correlated it to precariousness and highlighting parent’s key role. General opinion tended towards the creation of a new work tool, digital, as a smartphone application. It would structure the approach of excess weight, initial consultation and medical follow-up while integrating the teenager into the care. The content would take up the nutritional, physical activity, psychological, motivational, socio-behavioral aspects using explanatory videos. A general outline would be supplemented by objectives in a recreational framework as well as obesity « red flags ». The work instrument should be effective, practical, relevant and sustainable. Discussion: a well-thought-out smartphone application could be the right structural, motivational, and coordinating support for overweight management. Its success would depend on its appeal on teenagers ensuring also optimal ergonomics for caregivers.
