Recherche des freins à la prescription d'un traitement anti ostéoporotique, après la survenue d'une fracture sévère chez les patientes de plus de 50 ans passées par les urgences du CHU de Caen

Abstract : Introduction: Fracture osteoporosis, which is associated with an increase in morbidity and mortality, is a major public health problem. Numerous studies have shown that osteoporosis after fracture is under diagnosed and under treated. The main objective of this study was to find the obstacles to the prescription of an anti-osteoporosis treatment. Method: This is a retrospective, observational, single-center study carried out in the Caen University Hospital from 04/01/2020 to 07/31/2020. Patients over 50 with a severe osteoporotic fracture were included. The first data were collected via the hospital report. In order to collect the additional examinations conducted and the treatments received, the targeted patients were contacted via a telephone questionnaire. Their medical referent was contacted, when no care were provided. Results: 63 patients were included. Only 22.2% of patients received anti-osteoporosis treatment. A bone densitometry prescription was made for 36.5%. The main reasons for non-treatment were the high age associated with the presence of comorbidities and the lack of coordination between the city and the hospital. Conclusion: Fracture osteoporosis remains insufficiently managed both in outpatient and in hospital settings. The involvement of patients and their follow-up care by setting up a fracture liaison service seem necessary.
