Lithiases urinaires dans la spondylarthrite ankylosante : revue systématique de la littérature et méta-analyse

Abstract : Ankylosing spondylitis is a chronic inflammatory rheumatic disease characterized by axial skeleton damage and extra-articular manifestations such as uveitis, inflammatory bowel disease, psoriasis or cardiac diseases. Previous studies suggested higher prevalence of urolithiasis in AS whereas others showed a similar risk for urolithiasis in AS and in general population. The aim of the meta-analysis was to compare the prevalence of urolithiasis in AS and in the general population or in the chronic inflammatory rheumatic diseases. We performed a systematic literature review in PubMed base (last accessed October 25th 2021), which retrieved 549 abstracts. We included studies which compared the prevalence of urolithiasis in AS and healthy controls or CIRDs (rheumatoid arthritis and Behcet's disease). Eleven controlled studies were included in the meta-analysis. Data extracted were analyzed using Review Manager 5.4 (Cochrane). The results were expressed in odds ratio (OD) with confidence interval at 95% (95%Cl). We showed a higher prevalence of urolithiasis in AS patients than in healthy controls (OR=2.14, 95%CI[1.25, 3.68]) and a higher prevalence of urolithiasis in AS than in chronic inflammatory diseases (OR=4.03, 95%Cl[2.39, 6.78]). AS patients with history of urolithiasis displayed a significantly higher disease activity on the Bath ankylosing spondylitis disease activity index (BASDAI) and poorer spinal mobility measured by the Bath ankylosing spondylitis metrology index (BASMI), Schober's test and chest expansion. No difference was observed on disability measured by Bath ankylosing spondylitis functional index (BASFI). ln conclusion, AS is associated with a higher prevalence of urolithiasis. Mechanisms involved in this process remain unclear.
