Revue de la littérature : implantologie sous-périostée en secteur postérieur mandibulaire atrophique. Apport des technologies CAD/CAM

Abstract : Subperiosteal implants were first described in the 1940s allowing the rehabilitation of edentulous sector by placing a metal framework under the periosteum. They were gradually abandoned because of non satisfying clinical results and high complication rates. However, a renewal of interest for them is occuring, particularly for the posterior mandibular atrophic sectors. Indeed, the prosthetic rehabilitation of these heavily resorbed sites can be complex. Inferior alveolar nerve’s proximity and the presence of many anatomical structures can compromise the placement of endosseous implants and require an increase of bone height, which make the treatment plan trickier. The development of three-dimensional reconstruction technologies and CAD/CAM allows the creation of a custom-made subperiosteal implant respecting the vascular-nervous structures present in this region without the need for prior bone augmentation. This literature review aims to demonstrate in which case the use of subperiosteal implantology is a viable alternative for the rehabilitation of atrophic posterior mandibular sectors. It compares rehabilitation by subperiosteal implant to rehabilitation by endosseous implant with inlay graft in terms of success and reproducibility. It appears that despite the lack of publication concerning subperiosteal implants designed by CAD/CAM, it represents an interesting alternative to current techniques for the rehabilitation of posterior mandibular atrophic sectors.
