Risque de complications aiguës cardio-vasculaires lors de la mise sous traitement antihypertenseur « neutre » pour la recherche d’une hypertension artérielle secondaire

Abstract : As antihypertensive treatments can interfere with hormone dosage, a non-interfering treatment (NIT) is usually prescribed for secondary hypertension (HTN) investigations. NIT is said to be less efficient than other antihypertensive treatments; hence we assessed the occurrence of acute cardiovascular events while taking NIT for secondary HTN investigations. Patients were included if they had an initial non-NIT antihypertensive treatment, consulted in our HTN unit between January 2013 and December 2015, were given a prescription with no non-NIT treatment and were hospitalized afterwards. Primary outcome was defined as the occurrence of acute cardiovascular events while taking NIT. Secondary outcomes were defined as the occurrence of adverse events and hypokalemia while taking NIT, and the evolution of blood pressure (BP) between first consult and time of hospitalization under NIT. 158 patients were included. 4 of them (2,5%) presented with an acute cardiovascular event; they had been taking NIT for a longer time than other patients (92 days vs 27). BP levels stayed globally steady under NIT. However, in patients with increased BP under NIT, a decrease in antihypertensive treatment daily dose was more often seen (p = 0,014). 32% of patients presented with at least one mild adverse event. 45% presented with hypokalemia (76% of them had a history of hypokalemia, vs 35% in other patients, p<0,001). In conclusion, NIT should be prescribed at sufficient daily doses, for less than 30 days. Potassium supplementation should be added for all patients with hypokalemia history. Patients should be warned of possible occurrence of mild adverse events.
