Protocole de traitement des péri-implantites : approche chirurgicale

Abstract : Peri-implantitis is an inflammatory and infectious disease affecting both the peri-implant bone and mucosa. It is a multifactorial pathology triggered by pathogenic bacteria contained in the bacterial biofilm and influenced by many general and local risk factors. The diagnosis of peri-implantitis is clinical by visual inspection and peri-implant probing, radiographic by highlighting bone defects, biological by quantification of inflammatory mediators and bacteria. The treatment of this pathology is divided into several stages. First, etiological therapy aimed at controlling risk factors and non-surgical supragingival and subgingival debridement of the implant surface. Then, surgical treatment with the implementation of resective or regenerative surgery depending on the clinical case. Finally, the implementation of maintenance adapted to each patient and each situation. Multiple mechanical, chemical and electrolytic debridement protocols have been explored in the literature as well as various resective and regenerative surgery protocols. None have yet proven their superiority or achieved consensus. This work enabled the drafting of a resective or regenerative treatment protocol depending on the clinical situation.
