Évaluation du système d’insulinothérapie automatisée hybride DBLHU pour le traitement de patients avec diabète de type 1 hautement instable : analyse de la fréquence et de la nature des réglages paramétriques du dispositif

Abstract : Aims: In patients with highly unstable type 1 diabetes (T1D) and eligible for an islet or pancreas transplant, the closed-loop DBLHU (Diabeloop for Highly Unstable Diabetes) system was recently shown to be superior to an open-loop PLGS (predictive low glucose suspend) system. Furthermore DBLHU led to a durable improvement in real life. We aimed at studying the frequency and nature of the device settings modifications made to improve the metabolic control of the patients over the 8 months of closed loop system. Results: All seven randomised patients agreed to participate to this post hoc study. Over the 8 months of closed loop, DBLHu settings have been changed only 31 times for all the 7 patients. Most of the modifications took place in the first week of closed loop. Of the 34 specific DBLHU parameters, only 12 of them (35 %) were modified during the 8 months. Conclusions: number and frequency of DBLHU specific settings have been changed on seldom occasion, which opens up prospects for simplifying such a device for future marketing
