Prévalence et cinétique d’évolution de la dysfonction diaphragmatique échographique chez le patient âgé admis aux urgences pour détresse respiratoire aiguë

Abstract : Introduction : Respiratory distress (DRA) is one of the most common reasons for admission to the emergency room in patients over 75 years old. Diaphragmatic dysfunction (DD) could represent a triggering factor, a precipitating factor or even a full cause of respiratory failure in these patients. Material and method : Monocentric prospective observational study carried out within the Félix-Guyon hospital center between June 24, 2020 and April 6, 2021. The diaphragmatic excursion (DE) was measured in values assigned over 3 consecutive respiratory cycles in a patient in forward position at 30-45° in spontaneous ventilation (VS). The measurements were taken on D0, D1, D3, D7 and the day of hospital discharge. The DD was defined by an ED<1cm in men or <0.9 cm in women. We compared the evolution of the DE (in cm) of each diaphragmatic cupola over time and according to the presence or not of DD at the entrance (D0) using an ANOVA. Results : The prevalence of DD was 46% in these patients. The ED was significantly lower for the sides with DD on admission and on D1 compared to the group without DD. Discussion : First study on the prevalence of diaphragmatic function focuses on patients over 75 years old. The prevalence of morbidities and antecedents, particularly cardiovascular, was generally higher in our cohort compared to the general population of the same age. Conclusion: DD is common in patients over the age of 75 admitted to the emergency department for acute respiratory distress.
