Aspergillose pulmonaire invasive post pneumopathie sévère à SARS-CoV-2 : outils diagnostiques, étude des facteurs de risque, revue des cas de co-infection au CHU de Bordeaux

Abstract : COVID-19-associated pulmonary aspergillosis (CAPA) is an infectious complication affecting critically ill patients with acute respiratory distress syndrome (ARDS) following infection with SARS-CoV-2. This fungal superinfection requires early diagnosis and appropriate treatment to reduce mortality. This thesis work consists of two parts : first a review of the literature describing invasive pulmonary aspergillosis and COVID-19 as well as co-infections focusing on the pathophysiology and host risk factors of such superinfection. The second part is a review of CAPA cases at the Bordeaux University Hospital between March 2020 and August 2021, in particular a retrospective classification of these cases according to three algorithms: the EORTC updated in 2019, the AspICU of 2012 and the ECMM/ISHAM consensus of December 2020. We obtain between 24 cases according to the ECMM/ISHAM consensus, 13 cases according to the AspICU and 8 cases of CAPA according to the EORTC. The most common risk factors observed are high blood pressure, chronic obstructive pulmonary disease and heart disease. This study highlights the difficulty of diagnosing such a fungal infection and the need to explore the risk factors or protective factors of IA post ARDS COVID for precise screening of patients at risk in order to reduce mortality.
