Dépistage du cancer du col de l’utérus et offre de soins en France métropolitaine

Abstract : Background: the lack of health workers in France is a topical issue. Does the lack of liberal healthcare have repercussions on women’s health, particularly on cervical cancer screening? Several studies focus on the limitation factors of the pap smear test, either in terms of health professionals’ characteristics or patients’ characteristics. However, few studies were about the relation between pap smear realization and quantity of health-care provision (gynecologists, general practitioner, midwives) in France. Objectives: quantify the amount of liberal professional doing pap smear test , and study its relation with the smear test realized on the territory. Method: observational, descriptive and analytical quantitative study using the Assurance Maladie data on all the women eligible for cervical cancer screening, over the period 2017- 2019. A multivariate multiple linear regression analysis has been realized in order to take socio-economics criteria into account. Results: the 96 departments of Metropolitan France were analyzed. The cervical cancer screening rate was 58,4%. It was statistically associated with density of private gynecologists on the department (p=0.010), density of general practitioners(p=0.034) and density of freelance midwives (p=0.001). Conclusion: the lack of liberal health care provision in an area can generate a reduction of woman’s chances concerning the cervical cancer screening. The implementation of organized cervical cancer screening programs since 2019 may improve women’s education about cervical cancer screening, about the places where they can have their pap smear test, and therefore reduce the existing geographical inequalities in France.
