Colonisation à entérobactéries productrices d’une bêtalactamase à spectre étendu et consommation de carbapénèmes : étude rétrospective dans un service de réanimation

Abstract : Objectives: the objective of this study was to evaluate ICU survival according to carbapenem administration during the stay in patients colonized with ESBL. The secondary objectives were to analyze the same criterion in patients infected with ESBL, in immunocompromised patients, to evaluate the rate of ESBL infection and to identify the antibiotics administered. Methods: all patients admitted to the medical intensive care unit at Bordeaux University Hospital between January 2018 and August 2020 with documented ESBL colonization during their stay were included in the study, retrospectively. Results: two hundred and forty-two patients were included. Mortality was not statistically different between the two groups in the total population (p=0.07), in ESBL-infected patients (p=0.23) and in immunocompromised patients (p=0.819). ESBL infection was documented during the ICU stay in 15% of patients. 24% of patients received carbapenems. Conclusion: the survival in the ICU of patients treated with carbapenems was not different from that of patients who did not receive them. Screening should be combined with a strategy in case of colonization, the role of rapid detection tests of ESBL-E should also be evaluated.
