Évaluation d’une méthode de réorientation vers la médecine de ville, pour des patients non urgents consultant aux urgences du Centre Hospitalier Ouest Réunion (CHOR)

Abstract : Background: Non-urgent patients consulting at the emergency department (ED) have shown to participate in ED overcrowding. A redirection of these patients toward primary care services could improve the flow of patients in the ED. The main aim of this study is to evaluate the efficacy of this method, by calculating a redirection rate. Method : This observational, retrospective and monocentric study was conducted at the ED of the Centre Hospitalier Ouest Réunion (CHOR), on Reunion island, France. All patients over 18 years old, redirected from the ED toward a primary care service (PCS), from December 1 2020 to April 29 2021, were included in the study. Results : During the study period, 580 patients were redirected toward a primary care service, resulting in a redirection rate of 9%. Over the entire period, the redirection rate was calculated at 4%. The redirected patients were males (48%) or females (52%) with a mean age of 40 years, mostly self-referred (83%). The 3 main admission reasons were abdominal/epigastric pain, fever and vertigo/dizziness. Traumatology was concerned in 42% of cases. Emergency physicians were all satisfied with the method. Conclusion : A redirection procedure of non urgent patients from the ED toward a primary care service, results in a redirection rate of 9%, suggesting its significant effect in the reduction of patient flow in the ED.
