Place des coupeurs de feu dans la prise en charge ambulatoire et hospitalière des brûlures en Haute-Savoie en 2007

Abstract : INTRODUCTION: Burn healers (BH), who play a part in folk medicine, claim they can alleviate the pain due to burns by "talking the fire out" of burns thanks to a secret incantation. Some members of the medical staff, particularly in emergency rooms in Haute-Savoie, resort to BH for patients suffering from burns. GOAL: evaluate the role and the impact of BH in the medical treatment of burns. METHOD: 3 surveys among 1) the medical staff of three emergency departments (Haute-Savoie); 2) the patients suffering from burns who consulted the medical staff in these departments; 3) and the general practitioners (GPs) of Haute-Savoie. RESULTS: 134 medical staff, 173 patients and 210 GPs answered the surveys. 61% of the medical staff are rather favourable to BHs' intervention in the department, what they suggested to 75% of their patients, among whom 81% agreed. 87% of those patients say their pain decreased by more than 30%. GPs' opinion is more moderate. DISCUSSION : BHs' interventions do not decrease in Haute-Savoie. Patients have recourse to BH, and medical staff too. The placebo effect partly brings light to the understanding of this phenomenon. Every therapeutic relationship implies at least anthropological, psychological, relational elements and mysterious ones. Beyond evidence-based medicine, one way to explain the mysterious efficiency of BH is the therapeutic relation itself and its symbolical effect. CONCLUSION : is it necessary to include culture and meaning into medicine in order to develop a more global management of pathologies? Complementary studies would be necessary to identify any potential specific benefit brought by the intervention of burn healers.
