Application et cohérence locale des recommandations nationales dans l'antibiothérapie des infections urinaires de l'enfant

Abstract : The aim was to establish links between practising, local microbial ecology and national data, international scientific literature, concerning children urinary infection. That's why we have made two studies. The first one was an epidemiologic study talking about bacterial sensitivity with 89 urinary cultures. Sensitivity of Escherichia coli was good with 98.5% [IC95 : 95.6 à 100]. This low rate of resistance confirmed to keep on prescribing cefixime in pyelonephritis. Concerning cystitis, results revealed to prefer using amoxicillin-clavulanic acid. Its urinary concentration is over and above intermediate MIC in 85.4% [IC 95 : 78.1 - 92.7] of urinary cultures. The second study was a postal survey using a questionnaire about 42 doctors' practising. Concerning pyelonephritis, empirical antibiotic therapy with third-generation cephalosporin was often prescribed. Scientific literature highlights equivalence between parenteral and oral antibiotic therapy. For cystisis, we know a rate of cefixime prescription from 29% to 36%. First study's data revealed amoxicillin-clavulanic acid treated correctly cystitis. Cystitis' treatment was often longer than 3 days. Literature reports short antibiotic therapy is equivalent to a longer one. These studies revealed some practising's amelioration. Concerning pyelonephritis, oral empirical antibiotic therapy will be preferred. For cystitis, empirical antibiotic therapy with amoxicillin-clavulanic acid is enough, during 2 to 4 days long. Results will be passed on practitioners.
