Évaluation des cas de déficit en vitamine c et de scorbut dans des services de médecine de l'est de La Réunion

Abstract : Introduction: Some cases of symptomatic vitamin C deficiency were discovered in a department of internal medicine from 2010 to 2011, this work wants to confirm the presence of vitamin C deficiency in Reunion Island and to determin risk factors, clinical and biological signs of scurvy. Patients and methodes: We collect in a retrospective way serum ascorbic acid level (SAAL) in hospitalized patient or in consultation in a hospital of Reunion Island between the 1rst of January 2011 and the 31th of December 2012, and their risk factors, clinical and biological signs of scurvy and anatomopathologic skin biopsy report. Results: One hundred and fifty two patients were included, 106 had vitamin C deficiency (SAAL<12 μmol/L) and 13 presented scurvy. The sampling was realised because of clinical signs (80,3%), anemia (53,7%) and the association of diabete and tobacco consumption (11,9%), 3,9 % presented no signs of scorbut or risk factors. Risk factors for vitamin C deficiency were alcohol consumption (p=0,037), tobacco consumption (p=0,023), male sex (p=0,022) and biological inflammatory syndrom (p<0,01). The presence of biological and clinical signs of scurvy was not significative. There is a significative association between diabete and chronic kidney disease. Skin biopsies in 4 patients with a SAAL ≤ 4 μmol/L didn't find any pathognomonic signs of scurvy. Conclusion: Vitamin C deficiency and scurvy, his clinical manifestation, are present in Reunion Island. This disease is still present in developed country, even in a place where fruit rich in vitamin C are abundant.
