Évaluation des pratiques professionnelles sur la prise en charge de l’hémorragie du postpartum immédiat

Abstract : Objective : to assess the quality’s evolution of prevention and management of postpartum hemorrhages (PPH) in Aliénor d’Aquitaine maternity ward (CHU Bordeaux), between 2001 and 2015, based on CNGOF recommendations published in 2014. Methods : this is a professional practice assessment (clinical audit). Study subjects were patients presenting with postpartum hemorrhage after a vaginal birth. During the first clinical audit in 2011, 50 patient records were analyzed and an analytical grid developed to classify them. Following this analysis, midwife training was implemented (via PowerPoint presentations and simulation workshops). From March to August 2015, 90 records were recorded and analyzed following the procedure developed previously. Results : PPH prevention improved : 72% of patients in 2011 received oxytocin after delivery as compared to 92% in 2015. PPH management improved : PPH-specific surveillance sheet was found in 3 records in 2011 as compared to 30 records in 2015. The proper care was provided earlier on average as compared to 2011. Conclusion : we found an improvement of professional practices during and before PPH between the 2 clinical audits.
