Prise en charge de la rhinite allergique chez l’adulte par les médecins généralistes installés et les internes de médecine générale à la Réunion

Abstract : Introduction : allergic Rhinitis (RA) is a global public health problem with considerable socio economic costs. It would affect 500 million people, altering their quality of life. Main objective : To compare the management of adult RA declared by general practitioners (GPs) and internal medecine residents (IMGs) in Réunion. Method : a comparative descriptive survey by anonymous self-questionnaire sent by e-mail and postal mail, conducted between March and April 2016 in Réunion. Results : 54 MG and 55 IMGs participated. A quarter of GPs versus 4% of IMGs (p = 0.001) were aware of RA recommendations. An associated asthma was detected on average by only 38% of practitioners, more by questioning for IMG (86.8% versus 23.6%, p <0.0001) and spirometry for GPs (44, 9% versus 9.5%, p <0.0001). The GPs reported performing a supplemental assessment at the patient's request more frequently than IMGs (p = 0.003). Many unnecessary additional examinations were prescribed. For mild RA, IMG prescribed more oral antihistamines in monotherapy (41.8% versus 20.4%, p = 0.01) but did not comply with the recommendations (p = 0.13). For severe RA, GPs were more compliant with the recommendations using more combination therapy with nasal corticosteroids and oral antihistamines (57.4% versus 27.3%, p = 0.001). Knowing the recommendations of the MGs improved their knowledge. The completion of an internship in ambulatory medicine for IMGs changed their prescriptions for medicines, bringing them closer to those of general practitioners. Satisfaction with patient information and education increased with work experience, completion of an internship for IMG and knowledge of recommendations for GPs. Conclusion : work experience improved knowledge of RA recommendations, made drug prescriptions more consistent with those for severe RA, and increased satisfaction with patient information and education.
