Anomalies hématologiques rencontrées en soins primaires et réseau de soins ville-hôpital en hématologie

Abstract : Introduction: the development of new therapies in hematology moves the management of these patients to ambulatory medicine. General practitioners (GPs) find themselves increasingly exposed to the management of complications of these diseases and their treatments. Objectives: the main objective of this study is to determine the nature and frequency of hematological abnormalities encountered in primary care medicine. Secondary objectives are to identify tools in order to improve the coordination of care with hematology and to determine the expectations of GPs in terms of training in this area. Materials and methods: a prospective study in France with an online form from 13 June to 31 July 2016. Statistical analysis was made by using Excel® and BioStatGV®. Results: 576 responses from 35 departments were analyzed. The main hematologic abnormalities encountered in primary care medicine are, in decreasing order: anemia, hyperleucocytosis, lymphadenopathy, leukopenia, monoclonal gammapathy and thrombocytopenia. The main expectations are a shorter appointment settling time; a reference contact with a direct line for counseling and a more explicit procedure in the report in case of complications. 95% of GPs are favorable for the development of training in hematology in particular on the Internet. Conclusion: GPs are increasingly involved in the care of hematology patients. Good coordination between these actors is essential to ensure quality care.
