Différences de suivi gynécologique et visibilité des femmes ayant des rapports sexuels avec des femmes : enquête de pratique auprès de médecins généralistes rhônalpins

Abstract : Context: Women who have sex with women (WSW) are invisible in primary care. Although, they have specific health care needs to meet, especially regarding gynecological care. They can experience negative healthcare courses because of heterosexuality assumption, ignorance, and discrimination by healthcare professionals. Purpose: The main purpose was to identify among general practitioners (GPs), differences in health care offer and clinical practice for WSW’s and non-WSW in a primary care setting. Methods: A practice survey, shaped as a cross-sectional observational, descriptive and comparative study has been conducted between October 2018 and February 2019. 338 general practitioners from Rhône-Alpes area received an anonymous questionnaire with clinical case-vignettes. The main outcome was the percentage of GPs who perform a different gynecological follow-up for WSW and non-WSW. Results: 165 questionnaires were analyzed. 89.7% of respondents performed a different gynecological follow-up for WSW and non WSW. They handled less often topics such as: contraception needs, barrier protections use, screening of sexual transmitted diseases (STD). 91.5% of respondents were aware that they follow WSW in their office, but 2/3 of them never or rarely asked for sexual orientation. Conclusion: Most GPs know that they follow WSW but may misidentify these patients and their real care needs. WSW receive in consequence a different and poorer follow-up as non-WSW. Clinical guidelines would be useful, to improve and harmonize quality and experience of healthcare for WSW.
