Étude péri-opératoire du langage chez les patients porteurs d'une tumeur cérébrale

Abstract : Language assessment remains essential in neuro-oncology. Language deficits exist in patients with brain tumours and may persist after surgery. The objective of this retrospective study is the perioperative evaluation of the language of this population. The neurosurgery speech therapists at Pasteur Hospital in Nice administered a specific French language battery before and after the surgery, including naming, semantic matching (BETL), repetition (BIA) and verbal fluency (Cardebat) tests. We collected language test results from 42 patients with brain tumours before and after brain surgery. Then, we analyzed the relationships between the presence of language deficits and tumour aggressiveness; demographic data (age, sex, socio-cultural level) and tumour data (hemispheric and lobar tumour topographies, tumour types, evaluation times).
Our results showed that the average number of pathological language tests is lower between preoperative/postoperative immediate assessments and deferred postoperative assessment (20 to 6 pathological tests) (p = 0.0020). Thus, most patients regain normal language skills 1 month after surgery. No correlation was shown between the average of pathological language tests and tumour aggressiveness, demographic data and tumour data (p > 0.05). The results obtained are qualified by limitations : the heterogeneity of the distribution of the population in statistical analyses, the heterogeneity of tumour pathologies. These results encourage a similar future study with a larger number of subjects and with the analysis of tumor volume and injured white matter tracts as other factors influencing language.
