Influence du poids sur l’activité de la polyarthrite rhumatoïde : étude prospective sur douze mois

Abstract : Introduction : The purpose of the study was to assess the influence of weight on Rheumatoid arthritis (RA) over twelve months.
Methods : Patients were prospectively included from a single center cohort of RA (diagnosed according to ACR/EULAR 2010 criteria and active according to the EULAR criteria), between 2001 and 2017. They were at least 18, treated either by a synthetic DMARDS or/and biological DMARDS. Only few missing data were collected retrospectively. The clinical response was assessed after 12 months’ treatment. The primary study endpoint was a decrease of the DAS 28 superior or equal to 1.2. The secondary study endpoint was the clinical remission according to EULAR.
Results : Out of 425 patients monitored; 307 were included in the 12 months’ survey. The multivariate analysis has revealed that only the PCR was a predictive factor for a subjects response to treatment (OR adjusted = 0.98; p = 0.034). In the group with biological treatment, the therapeutic response was influenced only by female sex (p = 0.047, OR = 0.45 [0.20; 0.97]) and biological treatment, essentially Tocilizumab (p = 0.005; OR = 2.7 [1.37; 5.45]). Weight did not seem to influence the therapeutic response whatever the type of treatment (p = 0.136 in patients without biologic treatment, p = 0.724 in patients with biologic treatment). Among the patients with anti TNF alpha, there was no significant difference between the different kinds of treatments (p = 0.390).
Conclusion : In our study, weight does not influence RA activity and does not seem to be a predictive factor of treatment response.
