L'échographie en médecine générale, est-elle utile ?

Abstract : Background: The main goal is to determine whether the ultrasonography performed by the general practitioner alters the patient’s orientation. Method: Quantitative, retrospective, and monocentric study. Meetings with general practitioners who attended an ultrasound training applied to general practice in February 2018 (hosted by Collège des généralistes enseignants de l’océan indien, a Reunion Islandbased French doctors’ association). The last 10 ultrasounds of each practitioner are analyzed with a questionnaire. Doctors must answer the following question: "If you did not have access to ultrasound in the office, would you have changed the patient's orientation?". The main criterion "patient’s orientation" is a composite of the following secondary criteria (to which the physician must answer "yes" or "no"): "emergency dispatch", "medical imaging", "laboratory ","specialist"," if deep vein thrombosis (DVT) suspected, treatment with probabilistic anti-coagulation ". Results: Among the 38 practitioners who attended the training, 15 were included. 150 ultrasounds results have been analyzed. In 67% of the cases, ultrasonography modified the patient’s orientation by avoiding 56% of orientation in an imaging office, 7% at the laboratory, 6% at the specialists, 3% to the emergencies. Conclusion: This study is a further argument in favor of the utility of ultrasound in general medicine in that it has the ability to change the orientation of patients, particularly by avoiding orienting the patient to the imaging office.
