Résection complète du ligament croisé antérieur pour dégénérescence mucoïde : étude rétrospective de 24 cas

Abstract : Background : Patients presenting with mucoid degeneration of the anterior cruciate ligament (ACL MD) usually complain of posterior knee pain and flexion limitation. There is no consensus on the treatment of this benign affection.
Hypothesis : Complete resection of the ACL can treat posterior pain and range of motion limitations associated with MD without creating clinical knee instability.
Materials and Methods : Twenty-three patients (24 knees) aged 28 to 74 with a 0.4 sex ratio had a complete resection of the ACL between 2011 and 2017. They were evaluated in a retrospective monocentric study with a minimum 12-month follow-up. Primary criteria were posterior pain (numeric pain rating scale) and flexion gain. All patients were evaluated via subjective IKDC and KOOS scores.
Results : Preoperatively, posterior pain was evaluated at 6.5/10 and maximum flexion was 107 degrees. After a mean follow-up of 3 years, posterior pain was down to 1/10 (p = 0.0001). Flexion gain was 28 degrees (p = 0.0001) with 5 degrees of extension gain (p < 0.05). Subjective IKDC progressed from 38.8 to 71.9/100 (p < 0.001). KOOS score items Symptoms, Pain and Daily Life were above 80/100. Differential laximetry on TELOS™ was 4mm (range 0 to 13mm). Six patients (25%) developed a clinical instability, including one patient who required a secondary ACL reconstruction. No preoperative criteria was correlated with the development of postoperative instability. For 7 patients (29%), radiological osteoarthritis had progressed by one IKDC grade (p = 0.1916).
Discussion : Complete ACL resection for mucoid degeneration is an effective treatment on posterior pain and limited range of motion, significantly improving knee function. One in four patients developed clinical instability after ACL resection with a limited impact on daily life activities.
