Prise en charge coordonnée de la complexité en soins primaires et qualité de vie perçue : évaluation quantitative du protocole proposée par l'équipe pluriprofessionnelle SENS au sein d'une maison de santé pluriprofessionnelle (MSP) à La Réunion

Abstract : Introduction : Complex health care needs are a major public health problem. The prevalence estimated is about 25 % of patients in primary care, account for about 80 % of global health care spending. Complexity is defined by the French Social Security Fund (“Accord conventionnel interprofessional”). During the year 2017, the care team of the community health center SENS at La Réunion, in France, has developed a protocol to treat patients with complex health care needs, which inclued a clinical pathway drive by a case manager. Does this protocol improve patients’ quality of life ? Method : We designed a monocentric, prospective and quantitative study. Patients included were complex health care patients who were aged ≥18 years examined in our centre over the course of 9 months. Quality of life was mesured by Medical Outcomes Study short scale (MOS SF 36), at the beginning of treatment and after 3 months. Results expected were physical component summary (PCS) and mental component summary (MCS). Study used a Wilcoxon signed-rank test. Results : From a total of 20 patients included 100 % of them replied to the survey. Sample was composed of 18 femals and 2 men, aged between 19 and 72 years old, 40 % workers, 15 % precarious situation, 40 % multi-pathology. The principal type of complexity was psychology. Study shows an improvement significant of PCS (p < 0.0169) and MCS (p < 0.0036) of MOS SF 36 after 3 months of management by primary care team. Conclusion : This Protocol is effective and usefull in current practice for patient care team SENS to treat complexity.
