Le bilan partagé de médication : un nouvel outil dans la prise en charge du patient âgé et polymédiqué à l'officine

Abstract : Eight years after the publication of the HPST law, pharmacists have a new mission assigned : « the medication shared review ». These new interviews are intended for elderly patients with chronic disease. The aim is to avoid drug iatrogenesis by optimizing treatments and assessing compliance. This thesis describes the establishment of these reviews, completed by an investigation of sixth year pharmacy students in order to get feedback on their implementation in pharmacies where they realised their professional practice internship.
