Déterminants de l’envoi aux urgences de patients gériatriques par le médecin généraliste sous le motif : altération de l’état général

Abstract : Background: general condition impairment (AEG) is a term commonly used by general practitioners to refer elderly patients to emergencies. It favors errors of orientation in emergencies and extends the care of the elderly patient. Although this notion remains difficult to explain, it seemed appropriate to analyze the determinants that motivate the generalist to refer an elderly patient to AEG in emergencies. Method: we conducted a qualitative study by semi-directed interviews with 15 general practitioners in the Landes - Basque Country. Results: the most important determinants were represented by environmental factors with the role of the caregiver in the first place as much by its feelings as by the help it provided to the patient. The limitations of urban medicine such as organizational constraints and availability could also tip the scales in favor of hospitalization. From the medical point of view, the notion of urgency prevailed and it is the presence of acute decompensation or acute short-term risk that alerted the doctor. Second, given the nonspecific side of the term, it was intuition and experience that influenced the doctor's judgment. Conclusion: the determinants of an emergency dispatch regarding an AEG situation are not directly related to the situation of AEG itself but rather to environmental criteria. While these determinants point to the misuse of the term AEG for sending to emergencies, medical practices have evolved and the notion of a medical emergency is often present.
