Étude de la prise en charge non médicamenteuse de l'insomnie en établissements d'hébergement pour personnes âgées dépendantes

Abstract : Introduction: insomnia is a common health problem in nursing homes (prevalence of 28 to 74 %). Several non-pharmacological treatments are recommended as first-line and validated by the literature in the management of insomnia in institutionalized elderly people. The methods of application of these treatments in nursing home have never been studied The main objective of the study was to know the rate of caregivers declaring to propose non-pharmacological treatments in the treatment of insomnia among residents in nursing homes in La Réunion. Methods: it was an observational, descriptive study conducted in nursing homes in La Réunion, consisting of interviewing caregivers of different workplaces, working day or night on their practices via an anonymous questionnaire hetero-administered. The inclusion criterion was to be a caregiver member of a nursing home involved in care directly with residents. The primary end point was the declaration from the caregiver to use non-pharmacological treatments to treat insomnia. Results: one hundred and fifty-seven caregivers (17 service agents, 63 caregivers, 20 medical and psychological assistants, 4 social auxiliary, 9 gerontological care assistants, 28 nurses, 8 practitioner and 8 psychologists) contributed to the study from September, 2017 to November, 2018 which is equal to 29.5 % of the total number of caregivers of the 14 nursing home participating (out of a total of 17 in La Reunion). Eighty-six percent of caregivers (n = 135) reported using non-pharmacological treatments in the treatment of insomnia. This care was most often initiated by the caregiver. Conclusion: a large majority of caregivers report non-pharmacological treatments for the treatment of insomnia. This result could be completed by a practice survey within a nursing home. Developing teaching and prescribing non-medical care could help optimize these cares.
